Realizing the daily challenges one faces as an injured sick worker, Care for America’s Workers was formed to coordinate a range of services such as general care scheduling of skilled nursing, therapy services, social services, and personal care assistance to cover long-term supportive services that may be provided by family members or other clinical staff.
Our On-going Monitoring and Care Services
Under the process, your primary treating physician must prescribe and request authorization for these care services. The payer then determines through utilization review whether the services are reasonable and necessary to cure or relieve the effects of the injury. The type, frequency and duration of services that are appropriate depend on the injured worker’s functional status and type of impairment.

As the most important member of the Care Team, CFAW Care Coordinators will provide you:
- Health care visits in your home and/or in the office.
- Facilitate phone calls with your healthcare providers.
- Attend physician appointments with you to stay current on the treatment plan.
- Monitor your symptoms of illness.
- Assist you in getting to your health goals.
- Assist you with obtaining your medications.
- Assist you with obtaining your durable medical supplies and equipment.
- Assist you with obtaining needed home modifications for your improved quality of life.
- Help schedule appointments with your doctors and providers.
- Assist with getting you to your doctor and other providers you see for your healthcare.
